For the convenience of members of our community the Port Townsend Police Department provides the Citizen Self-Reporting Form for the reporting of non-violent incidences which have already occurred within the City of Port Townsend.

Detailed instructions are provided on the first page of the document. Once completed the forms should be dropped off at the Port Townsend Police Department front lobby, Mon-Fri 8am-5pm, or mailed to 1925 Blaine Street, Ste. 1, Port Townsend WA 98368.
